Extensive Porous-coated Total Hip Arthroplasty in Osteonecrosis of the Femoral Head

Abstract
Purpose: The purpose of this study was to document the clinical and radiographic midterm results of extensively porous, coated, cementless total hip arthroplasty in a consecutive series of Ficat and Arlet stage ¥² or ¥³ osteonecrosis
patients.

Materials and Methods: From June 1996 to May 2000, we reviewed one hundred forty-one (Ed or simply, 141) primary total hip arthroplasties, in 110 patients with a preoperative diagnosis of advanced osteonecrosis of the femoral head (Ficat and Arlet stage ¥² or ¥³), after more than two years follow-up. The clinical result was assessed using Harris Hip Score preoperatively, and at 6 weeks, 3 months, 6 months, and annually after operation.

Results: The average Harris Hip Score improved from 60.5 points preoperatively to 89.1 points at last follow-up. In the radiographic evaluation, 135 femoral components were bone-ingrown, five were fibrous stable, and one was unstable. In one acetabular component, revision surgery was performed because of acetabular osteolysis resulting from excessive polyethylene wear. In one femoral stem, revision remained under consideration at last follow-up due to subsidence of more than 5 §® with radiolucent line around the entire stem.

Conclusion: The midterm results of extensively porous, coated, cementless total hip arthroplasty showed excellent fixation and pain relief in patients with advanced osteonecrosis of the femoral head. Further follow-up is required due to existence of osteolysis.
